Enrollment Eligibility

All benefits described in this Benefit Program Overview are effective on first day of the month following 60 days of employment (with the exception of the Retirement Plan). Pursuant to the Cafeteria Plan (Section 125) the majority of the benefits are deducted on a pre-tax basis.

Medical Benefits

The medical plan is offered through Neighborhood Health Plan (“NHP”) or UnitedHealthcare (“UHC”) depending on the state and/or county you reside. Both NHP and UHC medical plans offer a “Core Plan” and “Buy-UP Plan” Features which includes Prescription Drug/Mail Order Benefits. The Buy-Up Plan allows for in-network and out-of-network benefits.

Dental Benefits

The dental plan offers a DHMO and PPO Plan offered through UnitedHealthcare.


Is an open access plan which does not require the selection of a primary dentist and services must be sought out from a UHC participating provider in the Solstice Network. This plan offers orthodontic services coverage for children up to the age of 19. This plan is only available for participants residing in the State of Florida.

PPO Plan

The PPO plan allows you to seek services from participating and non-participating providers with no referrals. This is an in-network and out-network plan and the provider network is from UHC. The PPO dental plan includes a Consumer MaxMultiplier Rollover Benefit.

Vision Benefits

The vision plan is offered through MetLife utilizing the VSP Network. Vision benefits are available every 12 months for exams, frames and lenses or contacts. The plan offer in-network and out-network plan features.

Short Term Disability

The Short Term Disability Plan (“STD”) is intended to protect your weekly earnings for a short duration in case you become ill or injured. STD benefits start on the 15th day of your disability and will pay 60% of your weekly salary up to a maximum weekly benefit of $1000.00.

Long Term Disability – Employer Paid

Long-term disability is intended to protect your base annual income for a long duration in case you become ill or injured. After you have been disabled for 90 days, the plan will pay a monthly benefit of 60% of your pre-disability base annual earnings up to a maximum of $6,000 a month until you are no longer disabled or Social Security Normal Retirement Age. This is employer paid-benefit, for which you will be automatically enrolled upon eligibility.

Basic Life Insurance/ AD&D Benefits

Provides with a life insurance policy up to $35,000. This policy also automatically includes an equal amount of accidental death and dismemberment (AD&D) coverage. This is an employer paid-benefit, for which you will be automatically enrolled upon eligibility.

Voluntary Life

Additional life insurance for you and your eligible family can be purchased upon meeting eligibility. Age reductions may apply to life insurance amounts. The spouse premium is based on the employee’s age.

Employee Assistance Program

The employee assistance program offers confidential guidance and resources for you or an immediate household family member:

  • In-person help for short-term issues, up to three sessions with a counselor per person, per issue, per year
  • Toll-free phone and web access 24/7
  • Unlimited phone access to legal, financial and work-life services
