US MED BENEFITS
Health benefits described in this Benefit Program Overview are effective on first day of the month following 60 days of employment. The majority of the benefits are deducted on a pre-tax basis, pursuant to the Cafeteria Plan (Section 125).
The medical plans offered are an HMO and NPOS “National Point of Sale” network plans depending on the state you reside in. These medical plans offer a “Core Plan” or “Buy Up Plan” features which include Prescription Drug/Mail Order benefits. The NPOS plans allows for in-network and out-of-network benefits.
HEALTH SAVINGS ACCOUNT
US Med offers a type of savings account that lets you set aside money on a pre-tax basis to pay for qualified medical expenses. By using untaxed dollars in a Health Savings Account (HSA) to pay for deductibles, copayments, coinsurance, and some other expenses, you may be able to lower your overall health care costs.
The dental plan offered are DHMO or DPPO Plan.
The DHMO dental plan is an in-network only plan. You may seek services must be sought out from within a specified network. This plan offers orthodontic services coverage for adults and children dependents. This plan is only available for participants residing in the State of Florida.
The PPO plan provides benefits for services received from in-network and out-of-network providers. The PPO plan allows you to seek care from participating and non-participating providers with no referrals needed. This plan offers orthodontic services coverage for children up to the age of 18. The dental PPO plan includes the Extended Annual Max option.
The vision plan offered utilizing the EyeMed Network. Vision benefits are available every 12 months for exams, frames and lenses or contacts. The plan offers in-network and out-network plan features.
SHORT TERM DISABILITY
The Short-Term Disability Plan (“STD”) is intended to protect your weekly earnings for a short duration in case you become ill or injured. STD benefits start on the 15th day of your disability and will pay 60% of your weekly salary up to a maximum weekly benefit of $1,000.00.
LONG TERM DISABILITY – EMPLOYER PAID
Long-term disability is intended to protect your base annual income for a long duration in case you become ill or injured. After you have been disabled for 90 days, the plan will pay a monthly benefit of 60% of your pre-disability base annual earnings up to a maximum of $6,000 a month until you are no longer disabled or Social Security Normal Retirement Age. This is employer-paid benefit, for which you will be automatically enrolled upon eligibility.
BASIC LIFE INSURANCE / AD&D BENEFITS – EMPLOYER PAID
Provides with a life insurance policy up to $35,000. This policy also automatically includes an equal amount of accidental death and dismemberment (AD&D) coverage. This is an employer paid benefit, for which you will be automatically enrolled upon eligibility.
Additional life insurance for you and your eligible family can be purchased upon meeting eligibility. Age reductions may apply to life insurance amounts. The spouse premium is based on the employee’s age.
